EmpowerLink provides planned nursing visits in the person’s home and community. We translate clinical instructions into a practical care plan, monitor changes, document each visit and keep the authorised care team informed.

Our nurses assess skin integrity, deliver treatment ordered by the authorised clinician, record wound progress and escalate signs of deterioration. The focus is safe continuity between hospital, GP, specialist and home.

EmpowerLink nurses provide respectful assessment, implement current catheter and stoma instructions, support skin integrity and help the person and their support network understand the agreed care routine.

Our nurses provide planned medication support within their scope and competency. Every accepted service is based on a current medication order, safe storage, the right equipment and a documented response to missed doses or adverse changes.

EmpowerLink provides planned enteral feeding and device care where the person has current instructions from the treating team. We focus on consistent technique, comfort, hygiene, documentation and early escalation of device or feeding concerns.

EmpowerLink considers referrals for established tracheostomy and home ventilation routines when the treating respiratory team has supplied current instructions, equipment requirements, escalation thresholds and an emergency plan.

EmpowerLink supports an established palliative plan in the person’s home, with attention to comfort, dignity, preferences, family communication and timely escalation. We work alongside—not in place of—the GP and specialist palliative care service.

EmpowerLink provides recovery-oriented, trauma-aware community nursing for people with established mental health care. Support can include wellbeing monitoring, medication routines, health education and coordination with authorised clinicians.

Planned blood glucose monitoring and insulin administration based on current medication orders, with clear documentation and escalation instructions.

Planned observations and care routines for established health conditions, with changes documented and escalated to the authorised treating team.

Time-limited nursing visits at home that follow current discharge instructions and connect the hospital handover with the ongoing treating plan.

Planned bowel care delivered within nursing scope and competency, based on a current health care plan and person-specific escalation instructions.

Support based on a current seizure management plan, medication orders and emergency instructions, with competency confirmed before service begins.

Minor works such as grab rails, non-slip ramps, bathroom adjustments, repairs, fencing, and other accessibility upgrades tailored to your care needs.

Intake reviews the participant-specific support plan, risks, required worker competencies and clinical oversight before high-intensity support is accepted.

Where a registered Support at Home provider formally engages EmpowerLink for an agreed service, that provider remains responsible for care management and program obligations.
