Funding, payment arrangements and service fit are confirmed during intake before care begins.

NDIS

EmpowerLink is a registered NDIS provider. Plan funding, clinical fit and service capacity are confirmed during intake.

Support at Home

If a participant’s registered Support at Home provider formally engages EmpowerLink for an agreed service, that provider remains responsible for care management, the service agreement and program obligations.

Private Clients

Self-funded nursing and support can be quoted after scope, fees, products or consumables, scheduling and service capacity are confirmed.

Clinical Nursing

Community Nursing Services

In-home nursing care provided by Registered and Enrolled Nurses, including chronic disease management, medication support, wound care, and monitoring of complex health needs.

Advanced Wound & Pressure Injury Care

Specialist nursing support for acute and chronic wounds, pressure injuries, post-surgical wounds, and skin integrity concerns.

Continence & Catheter Management

Continence assessments, catheter care, stoma care, and support with incontinence products tailored to individual needs.

Medication Administration & Injections

Safe medication support including oral meds, injections, insulin, and complex administration such as subcutaneous and intramuscular delivery.

PEG & Enteral Feeding

Daily support and hygiene management for enteral feeding (PEG/NG tubes), ensuring nutritional needs are met safely and comfortably.

Tracheostomy & Ventilator Support

Specialist respiratory nursing support, including suctioning, humidification, monitoring, and ventilator care in the home.

Palliative & End-of-Life Care

Holistic, person-centred support for individuals and families during advanced illness, with a focus on dignity, symptom management, and emotional wellbeing.

Mental Health Nursing

Clinical mental health support provided by qualified nurses to assist with medication compliance, wellbeing monitoring, psychosocial support, and care coordination.

Home Care

Personal Care & Self-Care Support

Assistance with showering, dressing, grooming, toileting, and mobility to support independence and routine in daily life.

Meal Preparation & Dietary Assistance

Support with preparing meals, managing dietary requirements, and ensuring nutritional balance tailored to individual needs and preferences.

Domestic Assistance

Help with light housekeeping tasks such as cleaning, laundry, linen changes, and household organisation.

Community Support

Community Access & Social Inclusion

Support to attend community events, medical appointments, shopping, recreational outings, and social programs safely and confidently.

Transport Assistance

Door-to-door transport for appointments, day programs, and community activities with travel support and supervision as needed.

Companionship & Emotional Support

One-on-one support from caring staff to reduce isolation and build confidence through meaningful social connection.

Support Services

Respite & Short-Term Care

Temporary in-home care to give family or informal carers a break, while ensuring your loved one is safe, supported, and engaged.

Recovery Coaching & Psychosocial Support

Support for people with psychosocial disability to build capacity, improve daily living, and achieve mental health recovery goals.

Skill Development & Daily Living Training

Support to build independence with tasks like meal planning, using public transport, budgeting, personal care routines, and household responsibilities.

Home Modifications & Maintenance

Minor works such as grab rails, non-slip ramps, bathroom adjustments, repairs, fencing, and other accessibility upgrades tailored to your care needs.

EmpowerLink provides planned nursing visits in the person’s home and community. We translate clinical instructions into a practical care plan, monitor changes, document each visit and keep the authorised care team informed.

Wound and pressure injury care

Our nurses assess skin integrity, deliver treatment ordered by the authorised clinician, record wound progress and escalate signs of deterioration. The focus is safe continuity between hospital, GP, specialist and home.

EmpowerLink nurses provide respectful assessment, implement current catheter and stoma instructions, support skin integrity and help the person and their support network understand the agreed care routine.

Medication and injections

Our nurses provide planned medication support within their scope and competency. Every accepted service is based on a current medication order, safe storage, the right equipment and a documented response to missed doses or adverse changes.

PEG and enteral feeding

EmpowerLink provides planned enteral feeding and device care where the person has current instructions from the treating team. We focus on consistent technique, comfort, hygiene, documentation and early escalation of device or feeding concerns.

Tracheostomy and ventilator support

EmpowerLink considers referrals for established tracheostomy and home ventilation routines when the treating respiratory team has supplied current instructions, equipment requirements, escalation thresholds and an emergency plan.

Palliative care

EmpowerLink supports an established palliative plan in the person’s home, with attention to comfort, dignity, preferences, family communication and timely escalation. We work alongside—not in place of—the GP and specialist palliative care service.

EmpowerLink provides recovery-oriented, trauma-aware community nursing for people with established mental health care. Support can include wellbeing monitoring, medication routines, health education and coordination with authorised clinicians.
