Website forms and analytics

Contact messages, feedback and job applications are submitted through Zoho Forms, a third-party form service configured for EmpowerLink. Zoho processes the information entered into those forms so EmpowerLink's authorised team can receive and respond to it. Résumés and other application attachments are handled through the configured Zoho form and account controls.

Comprehensive referrals are submitted through EmpowerLink's protected referral portal. Do not use a general contact or feedback form for diagnoses, medication details or clinical records. Contact intake first if you are unsure which pathway to use.

Optional Google Tag Manager or Analytics loads only after you choose “Allow analytics”. We do not intentionally send form fields, names, diagnoses, referral details, résumé data, calculator inputs or calculator results to analytics.

Cookies and consent

A necessary browser-storage value remembers your analytics choice. Embedded Zoho forms may use their own essential technologies to operate and secure the form. If you allow analytics, Google tags may process device and usage information according to the configured consent settings. The K10 and BMI tool pages do not load the Google Tag Manager container, even after consent. You can clear site data in your browser to reset the choice.
