Supports matched to the participant—not a generic package

Our team discusses the participant’s goals, routines, communication preferences, current care arrangements and the practical outcome the referral is seeking. Where nursing is requested, a nurse reviews the clinical information, treating-team instructions and required competencies before care begins.

Nursing and disability support have different responsibilities

Registered or enrolled nurses carry out accepted clinical work within their professional scope and assessed competency. Disability support workers assist with the agreed daily-living, personal-care and community tasks in the service plan. Intake records who is responsible for each task so clinical procedures are not quietly transferred to an unqualified role.

Funding and eligibility

  • NDIS supports must relate to the participant’s disability, goals and approved plan.
  • Funding type, available budget and service agreement requirements are confirmed during intake.
  • Health-system or other funding responsibilities may need clarification with the referrer and participant.
  • No funding outcome or service acceptance is guaranteed by an online enquiry.

Other ways to access care

Support at Home participants

If a participant’s registered Support at Home provider formally engages EmpowerLink for an agreed service under a documented third-party delivery arrangement, that provider remains responsible for care management, the service agreement and program obligations. EmpowerLink does not claim direct Support at Home provider approval.

Private clients

Private clients and families can request self-funded nursing or support services. Clinical fit, service capacity, fees, products or consumables and the service agreement are confirmed before care begins.

Does EmpowerLink provide both nursing and disability support?

Yes, where the requested services match EmpowerLink’s registration, current scope and workforce capacity. Nursing tasks and support-worker tasks are assessed and documented separately.

Can NDIA-managed, plan-managed and self-managed participants enquire?

Potentially. Intake confirms the participant’s management arrangement, relevant support budget, requested service and service agreement requirements before care begins.

Does an online enquiry guarantee that a service will be accepted?

No. Acceptance depends on the participant’s goals, requested tasks, risks, location, funding, timing and appropriately skilled workers or nurses.

Can a support coordinator or hospital team make the referral?

Yes, with the participant’s or authorised representative’s consent. Referrers should use the secure referral form for personal and clinical information.
