EmpowerLink provides community nursing and in-home support for NDIS participants and private clients. We also deliver agreed services where formally engaged by a registered Support at Home provider.

Our staffed office is in Caboolture. The team reviews each referral to confirm the requested support, service location, funding arrangements and current capacity before care begins.

Care built around the person

Planning starts with the person’s goals, routines and communication preferences. We agree on the scope of support and responsibilities, connect nursing care with treating-team instructions, and explain the next steps.
