Planned nursing care in the home and community

Detailed clinical pathways explain service scope and referral requirements for NDIS participants, eligible Support at Home partner-delivery arrangements and private clients.

Important: These pages support referral conversations. Care is accepted only after clinical assessment, document review, workforce confirmation and agreement with the person or authorised representative.

Community nursing

EmpowerLink provides planned nursing visits in the person’s home and community. We translate clinical instructions into a practical care plan, monitor changes, document each visit and keep the authorised care team informed.

Wound and pressure injury care

Our nurses assess skin integrity, deliver treatment ordered by the authorised clinician, record wound progress and escalate signs of deterioration. The focus is safe continuity between hospital, GP, specialist and home.

Continence, catheter and stoma care

EmpowerLink nurses provide respectful assessment, implement current catheter and stoma instructions, support skin integrity and help the person and their support network understand the agreed care routine.

Medication and injections

Our nurses provide planned medication support within their scope and competency. Every accepted service is based on a current medication order, safe storage, the right equipment and a documented response to missed doses or adverse changes.

PEG and enteral feeding

EmpowerLink provides planned enteral feeding and device care where the person has current instructions from the treating team. We focus on consistent technique, comfort, hygiene, documentation and early escalation of device or feeding concerns.

Tracheostomy and ventilator support

EmpowerLink considers referrals for established tracheostomy and home ventilation routines when the treating respiratory team has supplied current instructions, equipment requirements, escalation thresholds and an emergency plan.

Palliative care

EmpowerLink supports an established palliative plan in the person’s home, with attention to comfort, dignity, preferences, family communication and timely escalation. We work alongside—not in place of—the GP and specialist palliative care service.

Mental health nursing

EmpowerLink provides recovery-oriented, trauma-aware community nursing for people with established mental health care. Support can include wellbeing monitoring, medication routines, health education and coordination with authorised clinicians.

Clinical governance

What the intake team checks

  • Current orders, care plans and relevant assessments
  • Clinical risks, escalation steps and emergency arrangements
  • Required nursing skills, competencies, equipment and consumables
  • Consent, communication, cultural and environmental needs
  • Funding, scheduling and continuity requirements

EmpowerLink is not an emergency service. Call Triple Zero (000) for immediate danger or urgent medical help. For time-critical deterioration, follow the person’s clinical escalation plan.
