Requested support, desired outcome, diagnosis or disability context and the person’s own priorities.

Clinical picture

Current orders, assessments, care plans, recent changes, risks, equipment and escalation arrangements.

Practical fit

Location, start date, frequency, access, communication needs, funding and key contacts with consent.

What happens after intake contact

  1. Receipt and triage: the intake team checks whether there is enough non-sensitive information to discuss likely fit.
  2. Protected handover: intake confirms the approved channel before the referrer sends identities, diagnoses or clinical records.
  3. Clinical and operational review: we assess scope, risk, competencies, service area and current capacity.
  4. Outcome: we confirm whether assessment can proceed, outline costs and next steps, or explain why the request is not suitable.

Privacy

Contact intake first so the team can provide the approved protected channel for personal and health information. Do not send clinical records through website analytics, URL parameters or general contact forms.
